How to Schedule your REMS Scan appointment
Click on Book Now for your desired location. Available dates will show for that location. Click on the desired date. Available times will show for that date. Do not use the calendar arrows to navigate. If you do not see your desired time for that date, exit from the booking page and select a different date for your desired location.
To confirm your desired appointment date and time,
you must check the acknowledgement box indicating you have read the Consent Form before Confirming Booking. To review the Consent Form click on the hyperlink in the acknowledgment. You must review the Consent Form.
If these steps are performed correctly, confirmation of the appointment will show on your computer screen. You will receive confirmation of the appointment and payment via e-mail along with appointment reminders. If you do not receive e-mail confirmation, check your spam folder.
Please read instructions for preparing for your REMS scan. A link is in your appointment confirmation and reminders. Appointment reminders are sent one week prior to your appointment and again one day prior to your appointment. If you have not received reminders, check your spam folder. Instructions can also be found under Services tab under On the Day of Your REMS Appointment.
We do not accept insurance or assist with insurance reimbursement.
There are NO REFUNDS for cancelled appointments.
